Sad and dead lotl

I have had my axolotl for a few months now, and recently have been battling a fungus with her. She was tubed in cool, dechlorinated water with daily water changes and salt baths for 10 minutes daily for two weeks. I moved her back to the tank, temp 18C, parameters are pH 7.5, ammonia 0, nitrite 0, nitrate 25. She had no rocks in her tank and is a mini (about 3 years old). I come home today and she was in the corner of her tank, mouth open, white and unmoving. She is not reactive to anything, her back legs are extremely swollen and I think one of them has a fungus but I cannot get a clear picture of it to double check. Based on all of this I am quite certain she is dead, her body is firm. I feel genuinely sick thinking about touching her and moving her back into a tub, what should I do? And what do people to once their lotl has passed?

Eye issues? Need opinions and advice.

So a few weeks ago, I noticed one of my axolotls had fungus on his gills and seemingly on his eyes. I tested the water and there was a significant nitrate spike, due to debris and poop collecting in a small area that was usually hidden from view (I'm usually vigilant for taking out droppings and doing partial water changes, but like I said, this stuff was somehow collecting behind a plant).

To combat the fungus, I started giving the axolotl salt bathes, but it wasn't working, so I started fridging him as well as salt bathes, and this got rid of the fungus on his gills. I've been fridging him for a week now, but his eyes still look off... I don't think its fungus anymore.

Now the eyes look a little bigger, and are even slightly pointy, and I know for sure that's not what they looked like before. I've switched from salt bathes to tea bathes, but so far, no obvious change.

Does anyone know what's going on? Here's a picture I took recently while changing out his fridge container water.

Fish tank

Hey guys my tank is 4 foot long and 2 foot tall. Only thing I'm concerned about is it's only 1 foot front to back. Is this enough room for my axolotl. I really don't have a way of getting an upgrade right now but I can hope for the future if needed.

There is a tiny worm on the glass I was trying to film to see if it’s something bad. His toes are just rotting off - he also has white around his eyes. I’ve been doing water changes and feeding twice a week and then getting all debris out with a turkey baster. What else can I do?
