Bad Molt Death; Trying to Understand What Happened

I've had a Blue boy from a pet store for about a year and a half. His first tank crashed completely and i started a new one which was going well. I live in FL and refill the tank with store bought spring water which is very hard and slightly high ph. I've always had a nitrates problem, although the new tank was staying on the high side of safe. There are no fish in the tank, he was completely alone. There are little tiny swimming larvae that I din't know how to get rid of, they're either moths or mosquitoes. He got shrimp pellets which he ate 90% of the time. The tank is 20 gallons with a waterfall filter, small heater and bubbler ring. He had a cave and a fake plant.

Last night it seems he tried to molt. I found the back-plate and the claws in his house, but the front half and back half of him were in separate spots outside of it. It's really gruesome.. It looks like he was ripped in half. There's white fuzz where the split is. i have pictures if that's helpful.

I guess I'm just trying understand what happened? He was fine then gone. He'd gone thru two molts before with me no problem. I want to know if it was my fault and how to fix it before i think about getting another, or fish or something. I've looked around but there's so much conflicting information..
